Jennifer KollmarJennifer M. Kollmar, PT, DPT
Clinic Director

Jennifer Kollmar is a graduate of Touro College in 2009 with a Doctorate degree in Physical Therapy. She was hired at Peak Performance Physical Therapy in New York and was assigned as a staff physical therapist. She treated outpatient orthopedic patients to include post/ops, sprains, strains and over use injuries.

In 2011 Jennifer received a commission in the U. S. Navy as a Lieutenant. Her first assignment was at Bethesda, Maryland, what is now called Walter Reed National Military Medical Center, assigned as a staff Physical Therapist. Her patients included active duty military, veterans, retirees, their spouses and children. She also involved in the care and treatment of returning wounded warriors from the Afghanistan and Iraq wars.

In the summer of 2013, Jennifer was selected to open a brand new Physical Therapy department in Sasebo Japan from scratch. Assigned as the sole physical therapist on the base, she and her staff were responsible for the care and treatment of 25,000 personnel assigned to the U.S. Navy fleet, which consisted of 12 Naval Ships, 26 supporting commands and base personnel.

After a successful tour in Japan, she received orders in 2015 to the USS Dwight D. Eisenhower, stationed in Norfolk Virginia. Assigned as the sole staff physical therapist for the 5,000 sailors assigned to the ship. She spent 2 years on the aircraft carrier and completed a 7-month deployment to the Arabian Gulf.

Jennifer separated from the U.S. Navy in August 2017 and was hired at her current location as the Clinical Director, Bosque River Physical Therapy and Rehabilitation in Waco. She is certified in Trigger Point Dry Needling and has attended the Colonel Douglas A. Kersey Advanced Clinical and Operational Practice Course in San Antonio, Texas; which specializes her in operational physical therapy. She brings a wealth of knowledge and experience treating and diagnosing outpatient physical therapy patients and is excited to be in Waco, Texas providing care and treatment to the surrounding areas.

Yes. The choice of where you receive therapy is always yours. While the majority of our patients come to us by physician referral/recommendation, many clients request to see us due to our strong clinical reputation in the community.

We operate in a direct access state where, if your insurance plan authorizes self-referral to physical therapy, a physician referral is not necessary for you to be seen by a licensed therapist. Please check with your insurance though as many health insurance plans require a physicians referral for therapy to be covered.We also offer treatments, services and/or products that do NOT require a referral and are available on a self-pay basis.

As a service to our patients, we will gladly file claims with your insurance company on your behalf. In order to appropriately submit these claims, we will need to obtain a copy of your insurance identification card to ensure that we have the necessary information.

Your therapist will conduct an evaluation of your condition lasting between 60-90 minutes. The evaluation is necessary to create a program designed specifically to address your needs for a maximum recovery.

– Referral slip from your medical doctor
– Your medical insurance card
– Your ID
– A copy of your medication list including dosages, if you have it
– If you are getting therapy as a result of a worker’s compensation claim or an automobile accident, you will also need to bring your claim information, including claim number.

Please arrive 15 minutes early on your first visit to fill out any necessary paperwork.

We recommend comfortable clothing such as sweatpants or shorts, a T-shirt or tank-top and sneakers.

Each visit will last 60 – 90 minutes on average. Times vary due to the treatment and exercises being performed.

The regularity of your visits will be determined after your initial evaluation, but most patients have 3 visits each week scheduled.
